Scheduling Appointments:

We realize that our patients' time is valuable, and we make every effort to see patients at their scheduled time.  However, occasionally, our office falls behind schedule.  We ask for your understanding; we also ask that patients provide as much advanced notice as possible when running late for an appointment or when they need to cancel.  With this advanced notice, the time can be scheduled for another patient.

Emergencies:

Emergency situations can arise unexpectedly.  In the event of a serious problem, or a situation in which you are in doubt, call our office at 504-246-8896 or 504-246-2612.  One of our physicians is on call 24 hours to help with your emergency.

What to Bring to Your Appointment:

If you are a first-time patient, please plan to arrive early in order to complete a patient registration form and please bring the following items to your appointment:

• Your insurance card
• Physician referral forms, if required by insurance
• A list of current prescriptions and/or over-the-counter medications you are using, including dose and frequency
• Any recent test results
